About the role

Key responsibilities & impact
  • Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive healthcare needs
  • Develop individualized treatment plans including medication management, preventive services, immunizations, screenings, lifestyle counseling, chronic disease management, and outcome monitoring
  • Diagnose and manage common primary care conditions including hypertension, diabetes, hyperlipidemia, asthma, COPD, coronary artery disease, obesity, thyroid disorders, depression, anxiety, musculoskeletal conditions, and infectious diseases
  • Order, perform, and interpret laboratory studies, diagnostic imaging, electrocardiograms, point-of-care testing, and other diagnostic evaluations
  • Perform office-based procedures including minor wound care, incision and drainage, skin lesion treatment, and joint and soft tissue injections
  • Conduct wellness visits, preventive examinations, chronic care management, transitional care management, health risk assessments, and preventive screenings
  • Identify complex presentations requiring physician consultation and coordinate referrals
  • Collaborate with physicians and interdisciplinary healthcare teams to coordinate comprehensive patient care
  • Educate patients and caregivers on diagnoses, medications, preventive health, nutrition, exercise, and treatment plans
  • Document encounters, medical decision-making, procedures, treatment plans, quality measures, and outcomes in the electronic health record
  • Participate in quality improvement, population health management, patient safety, clinical pathway development, and continuing professional development
  • May oversee nursing and allied support staff
