About the role
Key responsibilities & impact- Screen and assess for mental health and substance use disorders
- Provide short-term, brief interventions using evidence-based approaches including BA, PST, and MI
- Support medication adherence and side-effect monitoring
- Provide education and self-management support
- Track clinical response through in-person, video, or phone contact and adjust plans as needed
- Refer patients for specialty care when appropriate
- Participate in weekly psychiatric case consultation
- Document in the EHR according to model workflows
- Re-engage patients who fall out of care
- Partner with primary care providers and psychiatric consultants to provide targeted care to adolescents in the Collaborative Care Program
Requirements
What you’ll need- Master’s degree in Social Work, Counseling, Psychology, or related field
- Clinical Social Work Associate (CSWA) license
- Experience working with adolescent populations
- Experience with EMRs and telehealth workflows
- Proficiency with technology and using multiple applications
- Located in Oregon
- Ability to work Monday–Friday, 8:00 AM to 4:30 PM
- Ability to work 2–3 days per week in Eugene, OR clinics
- Practice under supervision of independently licensed clinicians as required by Oregon Board regulations
