About the role

Key responsibilities & impact
  • Screen and assess patients for mental health and substance abuse disorders
  • Facilitate patient engagement, treatment plans, and follow-up care
  • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, and motivational interviewing
  • Support psychotropic medication management by monitoring treatment adherence, side effects, and effectiveness
  • Provide health behavior and social determinants of health interventions
  • Track treatment response and monitor patients in person, by video, or by telephone
  • Educate patients about common mental health and substance abuse disorders and available treatment options
  • Provide or facilitate referrals to evidence-based psychosocial treatments
  • Participate in weekly caseload consultations with the psychiatric consultant and communicate treatment recommendations to the medical provider
  • Track patient follow-up and clinical outcomes using the EHR
  • Document patient encounters, progress, and treatment recommendations in the EHR and other required systems
  • Identify and re-engage patients using established clinical workflows
  • Facilitate treatment plan changes for patients requiring more intensive or specialized mental health care
  • Participate in process improvement initiatives
  • Attend meetings and connect with clinicians and administrators to maintain collaborative workflows
  • Meet monthly performance metrics determined by the clinical manager and clinical director

Requirements

What you’ll need
  • Minimum of a master’s degree in Social Work, Psychology, Counseling, Rehabilitation, or another relevant field
  • 3-5+ years of experience in behavioral health intervention clinical work, case management, and/or community care coordination
  • Experience working with various populations, including older adult Medicare populations with chronic medical and behavioral health challenges
  • Active independent clinical license in Oregon
  • Eligible licenses include Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Psychologist
  • Current enrollment in the state’s Medicaid & Medicare is preferred, but eligibility is required
  • High-speed home internet
  • Proficiency with electronic health records (EHR), scheduling systems, remote phone systems, and video systems
