About the role

Key responsibilities & impact
  • Screen and assess for mental health and substance use disorders
  • Provide short-term, brief interventions using evidence-based approaches including BA, PST, and MI
  • Support medication adherence and side-effect monitoring
  • Provide education and self-management support
  • Track clinical response through in-person, video, or phone contact and adjust plans as needed
  • Refer patients for specialty care when appropriate
  • Participate in weekly psychiatric case consultation
  • Document in the EHR according to model workflows
  • Re-engage patients who fall out of care
  • Partner with primary care providers and psychiatric consultants as a core member of the Collaborative Care team

Requirements

What you’ll need
  • Master’s degree in Social Work, Counseling, Psychology, or related field
  • Clinical Social Work Associate (CSWA) license
  • 3+ years of experience working with adult populations with chronic medical and behavioral needs
  • Experience with EMRs and telehealth workflows
  • Proficiency with technology and using multiple applications
  • Located in Oregon
  • Ability to work Monday-Friday, 8:00 AM to 4:30 PM
  • Practice under supervision of independently licensed clinicians as required by Oregon Board regulations
