About the role

Key responsibilities & impact
  • Screen and assess patients for mental health and substance abuse disorders
  • Facilitate patient engagement, treatment plans, and follow-up care
  • Provide brief behavioral interventions using evidence-based techniques, including behavioral activation, problem-solving treatment, and motivational interviewing
  • Support psychotropic medication management through treatment adherence, side-effect, and treatment-effectiveness monitoring
  • Provide health behavior and social determinants of health interventions to improve functioning and medical outcomes
  • Monitor patients in person, by video, or by telephone for changes in clinical symptoms and treatment side effects or complications
  • Provide patient education about mental health and substance abuse disorders and treatment options
  • Provide or facilitate referrals to evidence-based psychosocial treatments
  • Participate in regularly scheduled caseload consultations with the psychiatric consultant and communicate recommendations to the patient’s medical provider
  • Track patient follow-up and clinical outcomes using the EHR
  • Document patient encounters, progress, and treatment recommendations in the EHR and other required systems
  • Use clinical systems and workflows to identify and re-engage patients
  • Facilitate treatment plan changes for patients not improving as expected, in consultation with medical and psychiatric providers
  • Participate in process improvement and identify opportunities for enhancement
  • Attend meetings and make approximately monthly in-person clinic visits to maintain collaborative workflows
  • Meet monthly performance metrics determined by the clinical manager and clinical director

Requirements

What you’ll need
  • A minimum of a master’s degree in Social Work, Psychology, Counseling, Rehabilitation, or other relevant field
  • 3-5+ years of experience in behavioral health intervention clinical work, case management, and/or community care coordination
  • Experience working with various populations, including older adult Medicare populations, who have chronic medical and behavioral health challenges
  • Active independent clinical license in Oregon
  • Eligible licenses include Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Psychologist
  • Current enrollment in the state’s Medicaid & Medicare is preferred but eligibility is required
  • High speed home internet
  • Proficiency with electronic health records (EHR), scheduling systems, remote phone systems, and video systems
  • Ability to work Monday-Friday, 8:00 AM to 4:30 PM
  • Ability to work virtually and in the clinic located in Corvallis, Oregon
