About the role

Key responsibilities & impact
  • Build and maintain relationships with government agencies, non-profits, faith-based organizations, and academic institutions
  • Coordinate mobile clinic site schedules and ensure outreach-location readiness
  • Conduct outreach and education in targeted communities to promote clinic services
  • Assist with patient intake, appointment scheduling, and follow-up coordination
  • Support patient engagement strategies for underserved populations
  • Represent the organization at community meetings, health fairs, and coalition events
  • Track outreach metrics, including events, participants, referrals, and partnerships
  • Execute targeted communication campaigns with community partners for the maternal health mobile clinic program
  • Participate in webinars and training sessions
  • Assist with operating the mobile health unit as needed
  • Travel locally up to 100% to support mobile clinic operations and community events
  • Perform frequent on-site work at health events, community meetings, and outreach locations
  • Work on and around the mobile unit, including event setup and patient engagement

Requirements

What you’ll need
  • Bachelor’s degree in public health, health administration, social work, or another related healthcare field
  • CHW certification required
  • Minimum 2 years of experience in outreach, community engagement, or healthcare navigation
  • Experience fostering partnerships with community-based organizations
  • Understanding of community health needs, specifically in underserved populations
  • Strong communication, organization, problem-solving, and teamwork skills
  • Ability to work flexible hours, including evenings/weekends as needed
  • Proficiency in Microsoft Office Suite
  • Ability to lift and carry materials weighing 15+ lbs throughout the workday
  • Bilingual (Spanish/English or other language) preferred
  • Ability and willingness to provide world-class service to all stakeholders
  • Must be authorized to work in the United States
