About the role
Key responsibilities & impact- Provide after-hours triage, clinical assessment, and interventions via phone
- Support patients and families with symptom management and end-of-life care needs
- Coordinate care by collaborating with physicians, facilities, and internal teams
- Dispatch visits and coordinate services with vendors as needed
- Deliver clear, compassionate education and support to families
- Accurately document all interactions, care plans, and outcomes in the EMR
- Review team documentation for accuracy and compliance
- Partner with Telecare staff to ensure positive patient outcomes
Requirements
What you’ll need- Active RN license (no current or pending restrictions)
- Hospice experience
- Home Care Home Base (HCHB) experience
- Strong clinical judgment and critical thinking
- Excellent communication and phone-based patient care skills
- Ability to multitask in a fast-paced, remote environment
- Strong customer service and conflict resolution skills
