About the role
Key responsibilities & impact- Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
- Develop, implement, and update individualized plans of care in partnership with patients and families
- Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
- Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations
- Coordinate care across the caseload and serve as the primary liaison between patients, families, physicians, and the care team
- Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
- Manage symptoms and provide support and guidance to patients and families during end-of-life transitions
Requirements
What you’ll need- Active Registered Nurse (RN) license in the state you work in
- Proficiency with electronic medical records (EMR)
- Strong organizational, communication, and critical-thinking skills
- Ability to work independently while maintaining high standards of care
- Compassionate, patient- and family-centered approach
- Valid driver's license and reliable vehicle with current insurance
