About the role
Key responsibilities & impact- Visit eligible patients in their homes and introduce them to the Monogram Health program
- Build trust through respectful, face-to-face conversations
- Listen to patient needs and answer questions
- Help eligible patients feel comfortable enrolling
- Work toward daily expectations for patient visits, meaningful conversations, completed enrollments, and other performance goals
- Route efficiently across assigned patients and complete scheduled visits
- Conduct consistent follow-up to support patient engagement and enrollment goals
- Keep accurate CRM records and track follow-up activity
- Remain accountable to monthly individual performance goals
Requirements
What you’ll need- High school diploma or GED
- Valid driver’s license
- Reliable transportation
- Willingness to travel throughout the assigned territory each day
- Comfort with high-volume, in-person, door-to-door outreach
- Strong communication, relationship-building, organization, and follow-up skills
- Self-motivation, resilience, adaptability, and comfort working independently toward measurable goals
- Bachelor’s degree or experience in field outreach, community engagement, enrollment, business development, account management, customer service, or another goal-oriented role involving direct customer interaction preferred
- Comfort using CRM tools to document interactions, organize follow-up activity, and track progress
- Healthcare experience is not required
