About the role
Key responsibilities & impact- Serve as liaison between patients and Pod clinicians
- Schedule patient visits within the structured visit cadence
- Ensure Post-Hospital Discharge Visits are scheduled within 72 hours of patient discharge
- Scrub clinician calendars for the following week to ensure quality visits and optimize field staff calendars
- Contact patients with missed visits within the last three days to reschedule visits
- Reassign visits for unavailable clinicians, including clinicians who are out of office, transferred roles, or left the company
- Perform delegated Pod assignments as needed
- Provide coverage to other Pods and regions based on business needs, with possible schedule changes
- Participate in Pod huddles with the Market Manager, MAC, and Pod leadership
- Troubleshoot field clinician scheduling concerns and perform patient engagement or re-engagement activities
- Report directly to the POD Market Manager and follow direction from POD leadership
- Interact with peers, other departments, Pod leadership, Monogram Health leaders, patients, and internal and external customers
Requirements
What you’ll need- High School Diploma or GED required
- Minimum of one (1) year customer service, medical, scheduling, or other related experience; or, six (6) months’ prior Monogram Health Engagement Specialist experience required
- Demonstrated verbal and written communication skills to communicate effectively and professionally with patients, clinicians, and others within and external to the Company
- Ability to complete tasks within established timeframes
- Demonstrated ability to multi-task and adjust to changing priorities
- Demonstrated technical proficiency using and navigating multiple systems, including Salesforce, Athena, Microsoft Office Suite, and Doxy
- Ability to use multiple computer screens
- Ability to use critical-thinking skills to solve basic to moderately complex system and/or staffing issues
- Must live in Eastern/Central TN, Virginia, or West Virginia
- Ability to work remotely
- Ability to work on site and adhere to attendance standards
- Visual acuity to read scripts, company literature, electronic files, and patient demographics
- Hearing ability to respond to phone calls and converse with patients, colleagues, and others
- Manual dexterity for repetitive keyboarding, data entry, patient call documentation, dispositioning, and computer dialing
