About the role

Key responsibilities & impact

Answer incoming phone calls using AIDET techniques and HEART behaviors
Manage data, work lists, team assignments, tracking reports, summaries, customer billing, and chart audits
Coordinate referrals, post-acute care choices, ancillary providers, follow-up appointments, and transportation
Network with community agencies and act as a liaison between the hospital and community agencies
Oversee customer satisfaction survey processes and reports
Investigate, coordinate, and facilitate patient eligibility benefits, including Medicaid and Medicare
Facilitate patient assistance applications as needed
Collaborate with interdisciplinary teams, patients, and families in care planning conferences
Support patients preparing to self-manage and escalate medical and social service issues to nursing and social work
Coordinate health-related services to increase patient access and support adherence to care plans, appointments, and portal usage
Educate members and families about unique needs, risks, financial issues, and coping issues
Provide options using knowledge of community health resources
Maintain supply inventory and order supplies and resources for remote monitoring programs
Follow regulatory and compliance standards and document all care provided
Facilitate after-hours communication between patients, providers, and Mosaic departments
Keep detailed interaction records for timely billing and reporting
Perform other duties as assigned

Requirements

What you’ll need

H.S. Diploma - Required
2 Years - experience in healthcare, managed care, hospital or clinical setting, or other comparable work experience - Required
Basic Life Support (BLS) - within 90 days of hire Required
Hand/eye coordination; speaks clearly; good hearing
Ability to use computers, printers and telephone equipment
Ability to work as member of team and develop trusting relationship quickly
Excellent verbal and written communication skills including sensitivity to other cultures and ethnicities
Ability to drive required
Ability to negotiate steps into and within patients' homes
Visual and hearing skills to obtain a driver’s license are required
Must have access to transportation for timely scheduled home visits
Travel to off-site locations may be required
