Registered Nurse, Care Manager

About the role
Key responsibilities & impact- Provide care management to a group of health plan members
- Create Person-Centered Service Plans (PCSP)
- Coordinate services and document care according to health plan standards
- Provide assessment, planning, facilitation, and advocacy through an interdisciplinary approach
- Implement quality and cost-effective plans of care based on UAS-NY assessments
- Collaborate with members, families, health care teams, medical providers, and community-based programs
- Oversee a caseload of managed care members
- Complete required regulatory documentation clearly, concisely, and timely
- Coordinate health plan support staff, including LPNs, Community Health Workers, and Care Coordination
- Provide guidance and education to support staff
- Participate in departmental and staff meetings and required training
- Adhere to state and federal privacy regulations, including HIPAA
- Communicate and collaborate with members, families, health plan staff, and providers
- Act as a liaison and member advocate
- Demonstrate cultural competence and respect members' cultural health beliefs, language needs, and support systems
- Perform other duties as assigned
Requirements
What you’ll need- License and current registration as a Registered Professional Nurse in New York State
- Associate degree required
- BSN preferred
- Minimum of one year of Nursing Experience required
- Long-term care and/or managed care experience a plus
- Knowledge of homecare, Medicare, and Medicaid a plus
- Care Management Certification a plus
- Bilingual skills a plus
- Ability to work independently and prioritize workload
- Computer literacy and working knowledge of basic computer skills and Microsoft Office programs
- Strong communication skills, both person-to-person, virtually, and by phone
- Valid NYS driver's license and ability to drive in all types of weather
- Ability to learn and routinely utilize the organization's software systems
- Ability to perform duties involving exposure or potential exposure to blood, body fluids, or tissues
- Ability to lift up to 20 pounds frequently and up to 50 pounds occasionally
