About the role

Key responsibilities & impact
  • Deliver direct hospital-level clinical care to patients in their private homes
  • Conduct multiple nursing visits per day
  • Use remote patient monitoring and virtual visits to deliver continuous care
  • Collaborate with APPs, physicians, support staff, and hospital personnel
  • Communicate patient condition changes, clinical updates, and progress toward goals
  • Administer medications and provide complex comprehensive care as ordered
  • Document patient care accurately and adhere to quality standards
  • Perform assessments, high-tech IV procedures, and use advanced medical equipment
  • Insert and care for catheters
  • Manage drainage tubes, wound vacs, complex wound/burn dressings, oxygen, durable medical equipment, and telemonitoring devices
  • Enter orders and obtain product information using a laptop
  • Perform other duties as assigned

Requirements

What you’ll need
  • License and current registration as a Registered Professional Nurse in New York State
  • Associates Degree required
  • BSN preferred
  • Experience as a clinician in home and community-based services is highly valued
  • Ability to provide exceptional care to individuals in private homes
  • Excellent written and verbal communication skills
  • Proficiency in Microsoft Office or other word processing, spreadsheet, and database management tools
  • Strong organizational skills and ability to prioritize tasks and manage multiple responsibilities
  • Resourcefulness in problem-solving, and ability to motivate and educate others
  • Ability to work independently and manage multiple assignments
  • Computer literate and working knowledge of basic computer skills and Microsoft Office programs
  • Ability to learn the organization's software system
  • Ability to push/pull an average of 50 lbs. and maximum of 150 lbs.
  • Ability to lift/carry an average of 25 lbs. and maximum of 50 lbs.
  • Ability to perform patient transfers, extensive bending, standing, kneeling, walking in various weather conditions, and navigate stairs
  • Ability to drive in all weather conditions
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Bloodborne Pathogens Exposure Category I
