About the role

Key responsibilities & impact
  • Conduct telephonic clinical consultations in accordance with Lumicera's Patient Management Program
  • Perform patient assessments and medication reconciliation
  • Identify adherence barriers, potential drug interactions, medication-related concerns, and clinical intervention opportunities
  • Provide education on disease state management, medication administration, storage requirements, treatment expectations, and side effect mitigation
  • Answer clinical questions from patients and caregivers
  • Escalate patient-specific clinical concerns through established processes
  • Document consultations, assessments, interventions, and clinical recommendations in pharmacy systems
  • Maintain department productivity, quality, and service-level standards
  • Support quality improvement initiatives by identifying, documenting, and reporting medication errors and patient safety concerns
  • Maintain current knowledge of specialty medications, disease-state treatment guidelines, and emerging therapies
  • Adhere to federal and state laws, pharmacy regulations, accreditation standards, and organizational policies
  • Perform other duties as assigned

Requirements

What you’ll need
  • Graduate of an ACPE Accredited School of Pharmacy with a bachelor’s degree in Pharmacy or PharmD required
  • Current, unrestricted licensure as a Registered Pharmacist in the state of residence, or the ability to gain reciprocity within 4 months, may be required based on state of residence laws or client contracts
  • Experience counseling patients and caregivers regarding medication therapy, treatment expectations, and disease state management required
  • Experience documenting patient assessments, clinical interventions, and medication-related recommendations in accordance with pharmacy practice standards required
  • Experience evaluating medication regimens to identify potential drug interactions, adherence barriers, and other medication-related concerns required
  • Participate in, adhere to, and support compliance program objectives
  • Ability to consistently interact cooperatively and respectfully with other employees
  • Must reside in one of the eligible states listed in the remote work notification
