About the role
Key responsibilities & impact- Provide both in-person and virtual patient care based on market-specific needs
- Work 8-hour shifts between 8:30am and 5:30pm CST or 10-hour shifts between 7:00am and 5:30/6:00pm CST
- Participate in a virtual weekend rotation every 4th weekend, Saturday and Sunday from 9:00am to 12:00pm CST
- Submit in-house and outside referrals based on patient needs
- Work independently and collaborate with an integrated care team including mental health and physical therapy providers
- Contribute to Nice Healthcare’s mission to improve how people access and experience healthcare
- Travel in the community and conduct in-person visits
Requirements
What you’ll need- Must reside in Little Falls, MN
- Must have a Master’s Degree in Nursing or a Physician Assistant Studies degree
- Must have CPR/BLS certification
- Must have national certification from ANCC/AANP or NCCPA
- Must have a current unrestricted license in Minnesota
- Ability to lift a 30 lb bag frequently
- Must have an insured vehicle in a safe and acceptable condition
- Clean driving record with a valid driver’s license
- Must maintain space to store supplies for in-person visits
- Comfortable using technology
- Able to work a full day behind the wheel and on the road
- Proficient in phlebotomy skills, or willing to learn through provided training
- Able to manage various patient needs and transition efficiently
