About the role
Key responsibilities & impact- Assess service needs
- Develop the New Jersey Individualized Service Plan (NJISP) for access to services
- Liaise with clients, families, programs, and service providers
- Establish achievable goals, outcomes, and service eligibility
- Develop the mandatory Person-Centered Planning Tool (PCPT)
- Monitor clients monthly
- Coordinate and facilitate off-site team meetings at Residential and/or Day Programs
- Identify service needs for a caseload of 10 to 20 consumers
- Help families match individuals’ wants and needs with appropriate skills, services, and providers in their communities
Requirements
What you’ll need- One year of proven experience working with adults with developmental disabilities
- Ability to take initiative, work independently, make executive decisions when necessary, and meet deadlines
- Excellent written and verbal communication
- Reliable car
- Flexibility to attend meetings during the day when needed
- Flexibility to travel within Middlesex and Somerset counties for field visits
- Covid vaccination required by the State of New Jersey
- Must pass criminal background and CARI check
- A bachelor’s degree in psychology, sociology, social work, or related field
- Must reside in the State of New Jersey
