About the role
Key responsibilities & impact- Assist clients in developing plans of care that support living at home with health and safety
- Conduct annual and as-needed home visits
- Assess clients’ care needs
- Develop tailored care plans reflecting client preferences and priorities
- Coordinate caregiving, nursing, meals, medical equipment, and other needed services
- Collaborate with community partners to create holistic, comprehensive care strategies
- Continuously monitor and adjust care plans throughout the year
- Ensure clients receive appropriate support
- Work with the Care Management Program Supervisor to develop a schedule balancing regular in-person office time and optional remote work
Requirements
What you’ll need- Four years of paid, full-time, on-the-job social service experience providing care management and long-term support and services to vulnerable adults, plus a high school diploma or equivalent; OR
- Three years of paid, full-time, on-the-job social service experience providing one-on-one, in-person customer service and developing care plans and caseloads, plus a high school diploma or equivalent, with bilingual or bicultural experience; OR
- Two years of experience with DSHS information systems used by Case Management staff and college courses, an associate’s degree, or equivalent; OR
- Two years of paid, full-time, on-the-job social service experience and a bachelor’s degree; OR
- One year of paid, full-time, on-the-job social service experience and a bachelor’s degree in social services, behavioral or health sciences; OR
- Social service experience and a master’s degree
- One year full-time equivalent verifiable internship or volunteer social services experience may substitute for one year of paid social service experience
- Demonstrated interpersonal communication, interviewing, problem-solving, organization, time management, and documentation skills
- Ability to assess client health and functional status, develop care plans, assess caregiver skills, and make recommendations
- Client advocacy skills and sensitivity to diverse groups
- Effective oral and written communication
- Ability to plan, prioritize, and work independently with good judgment and minimal supervision
- Ability to establish and maintain effective relationships with coworkers, community agencies, and the public
- Ability to defuse difficult situations with sensitivity and assertiveness
- Ability to maintain confidentiality
- Ability to engage with communities of color, people with disabilities, LGBTQIA+, and other marginalized communities
- Understanding of institutional barriers to services
- Commitment to fostering belonging for individuals of all ethnicities, genders, ages, and backgrounds
- Valid driver's license, vehicle available for work-related travel, and appropriate liability insurance
- May be expected to obtain AIRS Certification during the second year of employment
- May be expected to obtain T-CARE Assessor Certification within the first six months, based on agency need
- May be expected to complete additional training/certifications such as motivational interviewing, communicating in conflict, mental health first aid, or Community Health Worker training
