About the role
Key responsibilities & impact- Conduct annual and as-needed home visits to assess clients’ care needs
- Develop tailored care plans reflecting clients’ preferences and priorities
- Collaborate with community partners to create holistic, comprehensive care strategies
- Monitor and adjust care plans throughout the year
- Coordinate caregiving, nursing, meals, medical equipment, and other needed services
- Work with the Care Management Program Supervisor to develop a flexible schedule
- Provide support during increased workload, staff vacancies, or other absences
Requirements
What you’ll need- Demonstrated skills in interpersonal communication, interviewing, problem-solving, organization, time management, and documentation of activities
- Demonstrated ability to assess client health and functional status, develop appropriate care plans, assess caregiver skills, and make clear recommendations
- Demonstrated client advocacy skills and sensitivity to diverse groups
- Effective oral and written communication
- Ability to plan, prioritize, and work independently with good judgment and minimal supervision
- Ability to establish and maintain effective relationships with coworkers, community agencies, and the general public
- Ability to defuse difficult situations with sensitivity and assertiveness
- High level of confidentiality
- Ability to engage with communities of color, people with disabilities, LGBTQIA+, and other marginalized communities
- Understanding of institutional barriers to services
- Four years of paid, full-time social service experience in care management and long-term support/services to vulnerable adults, plus a high school diploma or equivalent; OR
- Three years of paid, full-time social service experience in the specified areas, a high school diploma or equivalent, and bilingual or bicultural experience; OR
- Two years of experience with specified DSHS information systems and college courses, an associate’s degree, or equivalent; OR
- Two years of paid, full-time social service experience and a bachelor’s degree; OR
- One year of paid, full-time social service experience and a bachelor’s degree in social services, behavioral or health sciences; OR
- Social service experience and a master’s degree
- Internship or volunteer work in social services may substitute for paid social service experience on a one-year full-time-equivalent basis
- Valid driver's license, vehicle available for work-related travel, and appropriate liability insurance
- May be expected to obtain AIRS Certification during the second year of employment
- May be expected to acquire T-CARE Assessor Certification within the first six months and/or complete additional training/certifications
