About the role

Key responsibilities & impact
  • Provide inpatient pharmacy services on nights, working a 7-days-on/7-days-off schedule
  • Rotate through central dispensing and decentralized clinical areas
  • Perform clinical services to optimize drug therapy and document clinical interventions
  • Provide medication therapy management, including pharmacokinetics, parenteral nutrition, serum creatinine, and therapeutic drug monitoring
  • Advise physicians on formulary medications and alternative therapeutic options
  • Provide drug information to physicians, nurses, and allied health professionals
  • Participate in code arrest response and ensure medication availability
  • Identify, prevent, and investigate adverse drug reactions and medication errors
  • Provide drug-related patient counseling information
  • Review and interpret physician orders for incompatibilities, interactions, duplicates, and contraindications
  • Contact nursing personnel and prescribers regarding questions on drug orders
  • Dispense and label oral medications and IV solutions
  • Use electronic health records, automated dispensing machines, compounders, unit dosing, tube systems, and clinical decision support/data mining tools
  • Process patient charges and credits
  • Assist with maintaining drug supplies and verifying physical inventory
  • Manage controlled-substance legal and professional controls, including documenting waste
  • Supervise and oversee licensed and non-licensed supportive personnel
  • Report adverse drug reactions, medication errors, and good catches
  • Assist with training new pharmacists and pharmacy technicians
  • Follow regulatory, organizational, hospital, departmental, safety, and patient-care policies
  • Report safety incidents and participate in safety-related training
  • Participate in quality improvement and clinical projects as an active member of the patient care team

Requirements

What you’ll need
  • Bachelor's Degree in pharmacy from an accredited college of pharmacy - Required
  • Arizona State Board of Pharmacy license that is active and in good standing - Required
  • BLS certification - Required
  • ACLS certification - Required
  • PALS certification - Required for positions working in Emergency and Pediatrics departments
  • 3 months to 1 year of experience - Preferred
  • Understanding of computers and competence in using computers and basic software programs
  • Must keep all certifications and/or licenses up to date and valid before expiration
  • Must complete mandatory company modules and required job-specific training within the specified timeframe
