About the role
Key responsibilities & impact- Assist with patient care under the supervision of the charge nurse, clinical manager, and/or director
- Maintain department cleanliness and clean up after treatments
- Assist patients with transfers, wheelchair transfers, compression garments, mobility, therapeutic exercises, toileting, dressing removal, and activities of daily living
- Observe skin integrity and report problem areas to supervising clinicians
- Assist with catheter and IV removal and phlebotomy within scope of practice
- Prioritize and problem-solve in response to changes in patient status or condition
- Collaborate with physicians and healthcare professionals to support appropriate staffing and quality patient care
- Assess patient care needs accurately and serve as a resource to other staff
- Assist physicians with order entry into the EMR as needed
- Follow isolation precautions, bloodborne pathogen procedures, and safety policies
- For Discharge PCT: assist patients in and out of vehicles, reinforce discharge instructions, coordinate with interdisciplinary teams and community agencies, document in the EMR, secure DME, and support access to health-related services
- Answer and prioritize administrative telephone calls and obtain accurate caller information
- Assist with orientation and training of new PCT staff
- Maintain unit documents, charts, and copies of charts; assist the charge nurse with clerical duties
- Document objective patient data, care provided, and abnormal findings reported to supervising clinicians
- Report safety incidents through the Midas/RDE tool and monitor the work environment for safety issues
- Comply with state and federal regulations, company policies, and required training
Requirements
What you’ll need- High School Diploma or GED
- Graduate of a Nursing Assistant Program
- Current, unrestricted CNA or LNA license in the State of Arizona
- Basic Life Support certification (BLS — American Heart Association only)
- Fingerprint Clearance Card application number required upon hire if employee does not have an Arizona LNA license
- Fingerprint Clearance Card required within 90 days from date of hire if employee does not have an Arizona LNA license
- Understanding of computers and competence using computers and basic software programs
- For Discharge PCT: ability to collaborate with interdisciplinary teams and community agencies in patient care and care management
- For Discharge PCT: ability to use EMR for documentation and communication
- For Discharge PCT: knowledge of available community resources
- For Discharge PCT: understanding of post-discharge resource planning
- Ability to maintain current and valid certifications and/or licenses
- Ability to complete mandatory modules and required job-specific training within specified time frames
