About the role

Key responsibilities & impact
  • Assist with patient care under the supervision of the charge nurse, clinical manager, and/or director
  • Maintain department cleanliness and follow treatment cleanup procedures
  • Assist with patient transfers, mobility, compression garments, activities of daily living, and therapeutic exercises
  • Observe skin integrity and report problem areas to supervising clinicians
  • Assist with catheter and IV removal and phlebotomy within scope of practice
  • Prioritize and problem-solve in response to changes in patient status
  • Collaborate with physicians and healthcare professionals to support quality patient care
  • Assess patient care needs and serve as a resource to other staff
  • Assist physicians with order entry into the EMR as needed
  • Follow isolation precautions and blood-borne pathogen safety procedures
  • Assist discharged patients, reinforce discharge instructions, coordinate discharge logistics, and help access health-related services
  • Collaborate with interdisciplinary teams and community agencies
  • Use the EMR for documentation and communication
  • Assist with securing durable medical equipment
  • Answer and prioritize administrative telephone calls and gather caller information
  • Assist with orientation and training of new PCT staff
  • Maintain unit documents and patient charts; copy charts as requested
  • Assist the charge nurse with clerical duties
  • Document objective patient data, care provided, and abnormal findings
  • Report safety incidents through the Midas/RDE tool and attend safety training
  • Comply with applicable regulations, statutes, company policies, and required training

Requirements

What you’ll need
  • High School Diploma or GED
  • Graduate of a Nursing Assistant Program
  • Current, unrestricted CNA or LNA license in the State of Arizona
  • Basic Life Support (BLS—American Heart Association only)
  • Fingerprint Clearance Card application number required upon hire if employee does not have an Arizona LNA license
  • Fingerprint Clearance Card required within 90 days from date of hire if employee does not have an Arizona LNA license
  • Appropriate clinical competencies for assigned roles
  • Understanding of computers and competence using computers and basic software programs
  • For Discharge PCT: ability to collaborate with interdisciplinary teams and community agencies
  • For Discharge PCT: ability to use the EMR for documentation and communication
  • For Discharge PCT: knowledge of available community resources
  • For Discharge PCT: understanding of post-discharge resource planning
