About the role
Key responsibilities & impact- Assist with patient care, documentation, and supply management under the supervision of clinical leadership
- Keep units supplied, clean, and organized
- Assist with patient transfers, mobility, activities of daily living, compression garments, dressing removal, catheter and IV removal, and phlebotomy within scope of practice
- Observe skin integrity and report abnormal findings to supervising clinicians
- Prioritize and problem-solve in response to changes in patient condition
- Collaborate with physicians and healthcare professionals to support appropriate staffing and quality patient care
- Assess patient care needs and serve as a resource to other staff
- Assist physicians with EMR order entry as needed
- Follow isolation, blood-borne pathogen, safety, and organizational policies
- For Discharge PCT: assist discharged patients, coordinate discharge logistics, reinforce instructions, assist with transportation, secure DME, and connect patients with resources and health services
- Use EMR for documentation and communication
- Answer and prioritize administrative telephone calls and collect accurate caller information
- Assist with orientation and training of new PCT staff
- Maintain unit documents and charts and assist the charge nurse with clerical duties
- Document objective patient data, care provided, and abnormal findings
- Complete required safety training, mandatory modules, and job-specific training
Requirements
What you’ll need- High School Diploma or GED
- Graduate of a Nursing Assistant Program
- Current, unrestricted CNA or LNA license in the State of Arizona
- Basic Life Support certification from the American Heart Association
- Fingerprint Clearance Card application number required upon hire if employee does not have an Arizona LNA license
- Fingerprint Clearance Card required within 90 days of hire if employee does not have an Arizona LNA license
- Maintain appropriate clinical competencies for assigned roles
- Understanding of computers and competence using computers and basic software programs
- For Discharge PCT: ability to collaborate with interdisciplinary teams and community agencies
- For Discharge PCT: ability to use EMR for documentation and communication
- For Discharge PCT: knowledge of available community resources
- For Discharge PCT: understanding of post-discharge resource planning
