About the role

Key responsibilities & impact
  • Assist with patient care, documentation, and supply management under supervision of the charge nurse, clinical manager, and/or director
  • Keep units supplied, clean, and organized
  • Answer telephones, respond to questions within capabilities, and perform data entry and data collection
  • Assist discharged patients, coordinate discharge logistics, and help patients access health-related services
  • Perform or assist with procedures within the PCT scope of practice
  • Work closely with interdisciplinary team members
  • Maintain department cleanliness and clean up after treatments
  • Assist with patient transfers, mobility, activities of daily living, compression garments, dressing removal, catheter and IV removal, and phlebotomy within scope
  • Observe skin integrity and report abnormal findings
  • Prioritize and problem-solve changes in patient status
  • Assist physicians with EMR order entry as needed
  • Support discharge instructions, secure durable medical equipment, and collaborate with community agencies
  • Answer, evaluate, and prioritize administrative telephone calls
  • Assist with orientation and training of new PCT staff
  • Maintain unit documents, charts, and copies of charts
  • Assist the charge nurse with clerical duties
  • Document objective patient data and care provided
  • Report safety incidents, attend safety training, and comply with regulations, policies, mandatory modules, and job-specific training
  • Float/Flex Pool PCTs may work on any hospital unit, change departments during a shift, work as a Patient Safety Attendant, and are required to take call

Requirements

What you’ll need
  • High School Diploma or GED
  • Graduate of a Nursing Assistant Program
  • Current, unrestricted CNA or LNA license in the State of Arizona
  • Basic Life Support (BLS—American Heart Association only)
  • Fingerprint Clearance Card application number required upon hire if employee does not have an Arizona LNA license
  • Fingerprint Clearance Card required within 90 days from date of hire if employee does not have an Arizona LNA license
  • Ability to maintain appropriate clinical competencies for assigned roles
  • Understanding of computers and competence using computers and basic software programs
  • For Discharge PCT: ability to collaborate with interdisciplinary teams and community agencies
  • For Discharge PCT: ability to utilize EMR for documentation and communication
  • For Discharge PCT: knowledge of available community resources
  • For Discharge PCT: understanding of post-discharge resource planning
