About the role

Key responsibilities & impact

Assist with patient care, documentation, and supply management under supervision of nursing leadership
Keep units supplied, clean, and organized
Answer telephones, respond to questions within scope, and perform data entry and collection
Assist with patient transfers, mobility, activities of daily living, compression garments, dressing removal, catheter/IV removal, and phlebotomy within scope
Observe skin integrity and report abnormal findings to supervising clinicians
Prioritize and problem-solve changing patient care needs
Collaborate with physicians, healthcare professionals, interdisciplinary teams, and community agencies
Assist with EMR order entry, documentation, and communication
Support discharged patients, reinforce discharge instructions, coordinate logistics, secure DME, and assist with accessing health-related services
Answer and prioritize administrative calls and maintain accurate caller information
Assist with orientation and training of new PCT staff
Maintain unit documents, charts, and copies; assist charge nurse with clerical duties
Report safety incidents, attend safety training, and monitor workplace safety
Float/Flex Pool PCTs may work on any hospital unit, change departments during shifts, serve as Patient Safety Attendant, and take required call

Requirements

What you’ll need

High School Diploma or GED required
Graduate of a Nursing Assistant Program required
Current, unrestricted CNA or LNA license in the State of Arizona required
Basic Life Support (BLS—American Heart Association only) required
If employee does not have an Arizona LNA license: Fingerprint Clearance Card application number required upon hire and Fingerprint Clearance Card required within 90 days from date of hire
Ability to obtain and maintain appropriate clinical competencies for assigned roles
For Discharge PCT: ability to collaborate with interdisciplinary team and community agencies with patient care and care management
For Discharge PCT: ability to utilize EMR for documentation and communication
For Discharge PCT: knowledge of available community resources
For Discharge PCT: understanding of post-discharge resource planning
Understanding of computers and competence using computers and basic software programs
Ability to comply with Arizona State Board of Nursing laws and regulations, company policies, mandatory modules, and job-specific training
