About the role

Key responsibilities & impact
  • Provide clinically expert virtual triage service
  • Assess patients' presenting problems or chief complaints
  • Use evidence-based triage protocols to identify acuity and develop care advice and next-step recommendations
  • Serve as a liaison between patients and physicians
  • Establish positive first impressions and act as a patient advocate
  • Triage incoming telephone calls and electronic messages, prioritizing and resolving them and involving providers as appropriate
  • Complete the communication cycle
  • Develop, implement, and update individualized plans of care based on identified patient care, learning, and needs
  • Ask key questions to assess patients' clinical conditions and adjust the approach as needed
  • Document all communications in the electronic medical record
  • Schedule patient visits with providers according to protocols
  • Follow department triage protocols
  • Assist with scheduling appointments with Northwestern Medicine providers
  • Assist in orienting new staff members
  • Assist with process improvement projects
  • Participate in departmental quality improvement activities
  • Use service recovery skills to resolve problems or service breakdowns

Requirements

What you’ll need
  • Graduate of accredited RN program
  • 2 years of ambulatory care experience
  • Registered Nurse License issued by State of Illinois
  • Current AHA BLS Provider certification
  • BSN preferred
  • Specialty certification upon eligibility preferred
  • Excellent customer service and communication skills
  • Ability to function as part of a highly effective team
  • Ability to work part-time overnight shifts, 20 hours per week
  • Holiday and weekend availability required
  • Three weekend shifts every two weeks (Friday, Saturday, or Sunday) required
  • Ability to work 8-hour shifts starting at 11:30 PM or 12-hour shifts starting at 7:30 PM
  • Successful completion of a background check may be required
