About the role

Key responsibilities & impact
  • Conduct comprehensive diabetes assessments and develop care plans
  • Provide personalized diabetes education to patients
  • Coordinate care with referring providers
  • Develop Diabetes Self-Management Education (DSME) through books, PowerPoint presentations and patient education handouts
  • Provide nutritional and physical activity guidance
  • Under physician supervision, provide education and guidance on medications, Self-Blood Glucose Monitoring (SMBG), Continuous Glucose Monitoring (CGM), and diabetes technologies including insulin pumps, In-Pen and patch insulin devices
  • Serve as diabetes expert to medical and hospital staff
  • Serve as an integral member of the ADA Recognition education committee

Requirements

What you’ll need
  • Bachelor's Degree Required
  • Registered Dietitian, Registered Nurse or Registered Pharmacist required
  • Current licensure in the State of Ohio required
  • At least 2 years of work experience in a diabetes specialty and/or patient education
  • Certified Diabetes Care and Education certificate preferred
  • Ability to teach in settings with wide learner needs
  • Computer literacy with knowledge of electronic medical records (EMR) and Microsoft Office software applications desired
