About the role
Key responsibilities & impact- Work with physicians, staff and other health care team members to identify at-risk populations, including financial hardships, lack of transportation, abusive situations and malnutrition
- Assess needs of identified patients and provide case management
- Recommend interventions and implement plans to assist patients
- Link patients with systems that provide needed resources and opportunities
- Coordinate community resources and maintain liaison with community agencies
- Serve as liaison between patients, staff and community resources
- Advocate for patients by coordinating services and involving patients in case plans
- Educate Faculty Physicians, Residents and Staff about the referral process and community resources available to patients
- Provide ongoing case management as needed
- Provide excellent clinical services
Requirements
What you’ll need- Minimum of a Bachelor’s Degree in Social Work
- State of Ohio licensure (LSW)
- Experience in a healthcare environment
- Experience coordinating available community resources and payer programs and limitations is required
- Flexible to meet the needs of the patients and providers
- Demonstrative effectiveness in conveying information both in writing and verbally
- High level of interpersonal skills to handle sensitive and confidential situations
- Demonstrated poise, tact and diplomacy
