About the role
Key responsibilities & impact- Manage a patient panel with a broad array of patient needs
- Conduct acute, chronic, and well visits
- Treat patients in-office or in testing centers
- Conduct occasional tele-health visits
- Participate in weekly Clinical Rounds and other continuous learning modalities
- Collaborate with in-office teammates through daily huddles and with virtual clinical teams
- Apply clinical training and perform in-office procedures
- Work out of a co-location with Hartford Health
- Provide care on a full-time, 32-hour-per-week schedule
Requirements
What you’ll need- Completed an accredited NP or PA program with a national certification
- In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting, OR 1 year in an outpatient primary care setting coupled with either a 1 year primary care fellowship or 1+ year in an urgent care setting
- State licensed in CT, obtained by your One Medical start date
- Passion for human-centered primary care
- Ability to successfully communicate with and provide care to individuals of all backgrounds
- Ability to effectively use technology to deliver high quality care
- Clinical proficiency in evidence-based primary care
- Desire to be an integral part of a team dedicated to changing healthcare delivery
- Openness to feedback and reflection
- Ability to confidently navigate uncertain situations with patients and colleagues
- Readiness to adapt personal and interpersonal behavior to meet patient needs
- Willingness to obtain additional state licensure and credentialing for One Medical virtual primary care in additional states
- Authorized to work in the U.S.
