About the role

Key responsibilities & impact
  • Manage care for high-risk members with chronic behavioral and health conditions
  • Collaborate with members, caregivers, physicians, and the healthcare team
  • Ensure timely access to necessary care, continuity across settings, shared decision-making, and connections to supportive services and community resources
  • Assess assigned patients using medical diagnoses, chart reviews, personal interviews, and care-team data
  • Provide social services to patients and families for an assigned medical service
  • Provide or supervise plan-of-care interventions, including resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling
  • Involve patients and families in developing and implementing care plans
  • Educate patients and families about diseases, treatments, social or financial situations, emotional stress, and other problems
  • Monitor care progress toward goals and implement interventions as needed
  • Conduct high-risk screening and provide crisis counseling, education, and resources regarding abuse or neglect
  • Document counseling and screening sessions
  • Liaise between patients/families, clinical staff, and other agencies
  • Advocate for patient concerns and escalate ethical dilemmas through appropriate channels
  • Complete reports based on mandated reporting requirements
  • Precept newly hired staff and provide field instructor services for social work interns
  • Drive performance improvement initiatives, including data collection, research, development, and continuing education programs
  • Lead Care Management team meetings and interdisciplinary rounds
  • Assist with assigned projects and complete the Leadership Academy as assigned
  • Perform other duties as assigned

Requirements

What you’ll need
  • Master of Social Work from an accredited institution required
  • At least 3 years of social work experience
  • Current Licensed Master Social Worker (LMSW) or under supervision for Licensed Clinical Social Work (LCSW) or LCSW from the Oklahoma State Board of Licensed Social Workers
  • Basic Life Support from the American Heart Association required
  • Knowledge of the principles and practices of casework
  • Knowledge of community resources
  • Strong analytical skills to interpret situations and determine appropriate action
  • Ability to explain information understandably to patients, families, and staff
  • Strong interpersonal, communication, and leadership skills
  • Strong knowledge of healthcare regulations, including CMS guidelines
  • Detailed-oriented with excellent organizational skills
  • Strong assessment, critical thinking, and problem-solving skills
  • Ability to assess, diagnose, and treat mental health conditions through counseling services
  • Knowledge of basic leadership skills
  • Strong ethical standards and professional integrity; knowledge of Code of Ethics
  • Ability to handle emotionally charged situations with compassion and professionalism
  • Minimum availability of 2 shifts per month for PRN position
