About the role
Key responsibilities & impact- Assess assigned patients’ conditions using medical diagnoses, chart reviews, personal interviews, and care-team data
- Perform psychosocial assessments and communicate findings to clinical staff
- Provide or supervise care-plan interventions, including referrals, crisis intervention, prevention, education, advocacy, bereavement, therapy, and support
- Partner with healthcare teams and involve patients and families in care-plan development and implementation
- Educate patients and families about diseases, treatments, social or financial situations, emotional stress, and other problems
- Monitor care progress toward goals and implement interventions as needed
- Conduct high-risk screening and provide crisis counseling, education, and resources regarding abuse or neglect
- Document counseling and education sessions
- Act as liaison between patients or families, clinical staff, and other agencies
- Advocate for patient concerns and escalate ethical dilemmas through appropriate channels
- Complete reports based on mandated reporting requirements
- Precept newly hired Social Worker Level I employees and BSW/MSW students when eligible
- Perform other duties as assigned
Requirements
What you’ll need- Master of Social Work from an accredited institution required
- Current Licensed Master Social Worker (LMSW), or under supervision for Licensed Clinical Social Work (LCSW), or current LCSW from the Oklahoma State Board of Licensed Social Workers
- Current Basic Life Support certification from the American Heart Association required
- Level 1: 0–3 years of work experience; healthcare experience preferred
- Level 2: At least 3 years of social work experience
- Level 3: At least 5 years of social work experience
- Analytical skills to interpret situations and determine appropriate action
- Ability to explain information understandably to patients, families, and staff
- Basic knowledge of healthcare regulations, including CMS guidelines
- Strong communication, interpersonal, and leadership skills
- Detailed-oriented with excellent organizational skills
- Knowledge of assessment, critical thinking, and problem-solving
- Ability to assess, diagnose, and treat mental health conditions through counseling services, with LMSW under supervision or LCSW
- Completion of clinical supervision requirements
- Strong ethical standards and professional integrity; knowledge of Code of Ethics
- Knowledge of principles and practices of casework
