About the role

Key responsibilities & impact
  • Support members participating in CalAIM Community Supports housing programs, including Housing Transition Navigation Services, Housing Tenancy & Sustaining Services, and Housing Deposit assistance
  • Conduct outreach, intake assessments, comprehensive member assessments, in-person visits, follow-ups, and home inspections
  • Provide person-centered housing care management to high-needs members, including individuals experiencing severe mental health conditions and substance dependencies
  • Develop and manage individualized housing care plans
  • Coordinate appointments, healthcare, behavioral health, community services, and long-term housing support systems
  • Manage housing resource application pipelines, Housing Deposit Services packages, required documentation, and landlord allocations
  • Collaborate with housing authorities, landlords, shelters, property managers, healthcare providers, payers, and community organizations
  • Monitor housing stability and proactively address barriers to housing or health outcomes
  • Build trusting relationships with members and families; provide phone, video, and in-person support and advocacy
  • Represent Pacific Health Group at community outreach events and local initiatives
  • Build community partnerships, referral routes, and access to services
  • Navigate multiple platforms and complete accurate, timely, high-fidelity chart notes and clinical documentation
  • Ensure compliance with Medi-Cal, CalAIM, and Community Supports requirements
  • Track member progress, outcomes, and service delivery
  • Participate in interdisciplinary team collaboration, huddles, peer cross-coverage, and Lead Care Manager shadowing sessions
  • Identify service gaps and trends and collaborate with leadership to improve workflows

Requirements

What you’ll need
  • Strong understanding of the housing system and demonstrated working knowledge of local housing resources, housing navigation, community-based support programs, or transitional housing support workflows, such as Medi-Cal, CalAIM, Community Supports, or HUD programs
  • 3–5 years in case management, social services, or healthcare
  • Experience with Medi-Cal, CalAIM, and Community Supports programs preferred
  • Strong understanding of healthcare systems, managed care, and community resources
  • Excellent communication, empathy, and cultural competence
  • Strong organizational and time management skills
  • Proficient in case management systems, EHR platforms, documentation tools, and communication platforms
  • Ability to document in real time and manage member data accurately
  • Ability to travel locally within the hiring county
  • Valid California Driver's License and active auto insurance meeting CA requirements
  • Reliable personal vehicle for daily work use
  • Successful completion of a background check, including MVR
  • Able to travel up to 60–70% within the county for in-person visits
  • Successful completion of a Testlify skills assessment
  • Reliable working laptop for the first 21 days of employment until a company-issued laptop is received
  • Internet speed of 300+ Mbps download and 25+ Mbps upload
  • Effective time management skills
