About the role

Key responsibilities & impact
  • Provide supervision for Nurse Practitioners delivering multidisciplinary care
  • Provide virtual care to Medicare patients with chronic, complex, and overlapping medical, behavioral health, and social needs
  • Enable virtual healthcare for underserved patients using internal care-delivery technology and clinical judgment
  • Develop and refine clinical programs addressing patients’ holistic needs and care barriers
  • Collaborate with physicians, nurse practitioners, care managers, social workers, community health workers, and other care-team members
  • Support care coordination and address unique patient needs
  • Provide input on clinical programs under development
  • Help launch the Medicare program across multiple states
  • Report to physician leadership

Requirements

What you’ll need
  • 2+ years of clinical experience serving patients with complex social and healthcare needs
  • Experience and desire to work closely with a multidisciplinary team
  • Technology-savvy and experienced in digital-first healthcare, such as telemedicine
  • Comfortable working across different technology platforms
  • Physically located in California with a quiet, HIPAA-compliant, internet-connected workspace
  • Board certification or eligibility as a Family Physician
  • Eligible to enroll in Medicare
  • Able to work with patients across the lifespan, from pediatrics to geriatrics
  • Experience working with Medicaid/FQHC clinics serving underfunded or underserved populations
  • Experience working with Medicare beneficiaries managing chronic disease
  • Medical license in California, plus additional state licensure or willingness to obtain additional licenses
  • Active California DEA or willingness to obtain one
  • Startup experience is a plus
  • Must live and work in the United States
  • Availability for PST hours for urgent needs/support
  • E-Verify employment eligibility verification for new hires
  • Passing a pre-employment background check required
