About the role
Key responsibilities & impact- Evaluate potential clients as total persons and identify functional limitations impeding independent living
- Conduct in-depth assessments and reassessments covering psychosocial, rehabilitation, and environmental concerns
- Refer clients to special consultants for detailed evaluations and recommendations
- Develop care plans, implement services, and conduct follow-up and monitoring
- Identify and develop support systems for clients
- Consult with the Health Practitioner
- Complete case recording and reporting
- Complete client health and psychosocial assessments using relevant information sources
- Participate as an active team member in care planning
- Implement interventions and services using community resources
- Maintain and document regular client contact
- Identify, assess, and respond to crisis situations with appropriate interventions
Requirements
What you’ll need- Bachelor’s degree required in Social Work, Psychology, counseling, rehabilitation, gerontology or sociology
- Two years of experience working with the elderly
- Clinical assessment skills
- Knowledge of the community
- Ability to work with multidisciplinary team and to work with clients/families in the home setting
- Knowledge of aging and programs for seniors
- Must observe all legal, departmental, and/or hospital regulations
- Must be authorized to work in the United States
