About the role

Key responsibilities & impact

Perform telephonic member outreach and face-to-face encounters to facilitate program enrollment
Conduct departmental assessments and evaluate members holistically to identify needs, health goals, and barriers to wellness
Develop, implement, and revise member-specific plans of care and care manager interventions
Complete documentation according to departmental policy and regulatory standards
Provide targeted health education and proactive condition-management strategies
Communicate with providers and vendors involved in member care
Coordinate safe transitions to the next level of care, including post-discharge instructions, services, and follow-up
Implement strategies to prevent readmission
Collaborate with interdisciplinary care teams to improve member outcomes
Attend and present high-risk members at interdisciplinary rounds
Maintain professional growth through self-directed learning

Requirements

What you’ll need

Registered Nurse with current unrestricted license in state of residence
May be required to obtain other state licensure in states where Point32Health operates
Understand and follow the provisions of state-specific Nurse Practice Act(s) where Point32Health operates
National certification in Case Management desirable
Bachelor’s degree or relevant equivalent experience
5 years’ relevant clinical experience
Experience in home care or case management preferred
Proficiency in a second language desirable
Experience in specialty areas a plus
Skill and proficiency in Microsoft applications, technical concepts and principles, and computer software applications
Skilled in assessment, planning, and managing member care
Advanced communication and interpersonal skills
Ability to address multiple complex issues
Ability to organize and prioritize work and member needs
Strong clinical and critical thinking skills
Regard for confidential data and adherence to corporate compliance policy
Ability to make face-to-face visits as needed
Valid driver’s license and vehicle in good working condition
May be required to work additional hours beyond standard work schedule
