About the role
Key responsibilities & impact- Engage members facing medical utilization disorders and build supportive relationships
- Conduct holistic health assessments and identify social determinants of health and cultural influences
- Collaborate with members, caregivers, and providers to develop member-centric case management plans
- Set goals, address barriers, and support improved health outcomes
- Monitor, evaluate, and update case management plans
- Facilitate communication and coordination among the healthcare team
- Provide interventions using motivational interviewing and stabilize natural support systems
- Ensure caregiver needs are identified and met
- Maintain concise, accurate, and timely documentation meeting accreditation, contractual, and legal requirements
Requirements
What you’ll need- Active WA State or Compact RN or ARNP license that allows independent assessments
- 3 years of post-licensure direct care experience
- 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time
- CCM certification preferred; if not held at hire, required within 24 months of hire
- Bachelor’s or Master’s degree preferred
- 4 or more years of direct care experience across multiple patient populations, conditions, or care settings preferred
- Motivational interviewing experience preferred
- Chronic Condition Certifications preferred
- Integrated Case Management certificate preferred
- Chemical Dependency Professional certification preferred
- LEAN training preferred
- Individuals residing outside Washington must have or obtain the appropriate Washington State license, applying within 10 days of acceptance
- Internet access with minimum download speed of 75 Mbps
