About the role
Key responsibilities & impact- Coordinate an interdisciplinary plan of care or program for a specific patient population
- Function as a clinical, patient, and community resource, educator, and consultant
- Participate in direct and indirect patient care, patient rounds, consultation, multidisciplinary care planning, and evaluation
- Collaborate with health team members to develop, implement, and evaluate cost-effective quality programs
- Develop, provide, and evaluate educational programs for patients, families, staff, and the community
- Monitor and ensure accurate patient-record documentation
- Maintain required reports, records, and program statistics
- Monitor care quality, identify issues, and integrate process improvement into practice
- Participate in research/investigative studies when indicated
- Perform other duties as assigned
Requirements
What you’ll need- Associate degree in Nursing required
- Bachelor's degree in Nursing preferred
- Three (3) years of nursing experience
- Current RN compact/multistate license recognized by the NCSBN Compact State, or RN license to practice in the state where working
- Computer skills
- Mathematical skills
- Working knowledge of patient equipment (vitals, suction, defibrillator)
- Compliance with Prisma Health's purpose and requirements
- Ability to work day shift
