About the role
Key responsibilities & impact- Complete comprehensive nutrition assessments, reassessments, and follow-ups for skilled nursing residents
- Identify residents at nutritional risk and develop nutrition interventions and recommendations
- Monitor residents with weight changes, pressure injuries, altered skin integrity, poor oral intake, enteral nutrition, renal disease, diabetes, and other conditions affecting nutritional status
- Review weight trends, laboratory values, meal intake, supplements, medications, and other clinical information
- Develop and update individualized nutrition care plans, goals, and interventions
- Recommend therapeutic diets, oral nutrition supplements, food preferences, and other nutrition interventions
- Communicate nutritional-status changes and recommendations to nursing, physicians/providers, dietary staff, and interdisciplinary team members
- Complete timely documentation in the facility’s electronic medical record
- Participate in interdisciplinary care plan, weight, wound/skin, and other clinical meetings
- Provide nutrition education to residents and/or family members
- Collaborate with the Food Service Director/CDM and facility team regarding resident nutrition needs
- Assist with regulatory compliance related to nutrition services and survey preparation
- Maintain resident confidentiality and adhere to facility policies, procedures, and residents’ rights
- Perform additional nutrition-related duties as assigned
Requirements
What you’ll need- Registered Dietitian credential through the Commission on Dietetic Registration (CDR)
- State licensure/certification as required
- Clinical nutrition experience; skilled nursing or long-term care experience strongly preferred
- At least 1 year of clinical experience preferred
- Familiarity with nutrition assessment and care planning for geriatric and medically complex residents
- Experience with electronic medical record/charting systems
- Understanding of interdisciplinary care in a skilled nursing environment
- Strong clinical judgment, organization, and time-management skills
- Excellent written and verbal communication skills
- Ability to work independently while collaborating effectively with the facility’s interdisciplinary team
