About the role

Key responsibilities & impact
  • Complete comprehensive nutrition assessments, reassessments, and follow-ups for skilled nursing residents
  • Identify residents at nutritional risk and develop nutrition interventions and recommendations
  • Monitor residents with weight changes, pressure injuries, altered skin integrity, poor oral intake, enteral nutrition, renal disease, diabetes, and other conditions affecting nutritional status
  • Review weight trends, laboratory values, meal intake, supplements, medications, and other clinical information
  • Develop and update individualized nutrition care plans, goals, and interventions
  • Recommend therapeutic diets, oral nutrition supplements, food preferences, and other nutrition interventions
  • Communicate nutritional-status changes and recommendations to nursing, physicians/providers, dietary staff, and interdisciplinary team members
  • Complete timely documentation in the facility’s electronic medical record
  • Participate in interdisciplinary care plan, weight, wound/skin, and other clinical meetings
  • Provide nutrition education to residents and/or family members
  • Collaborate with the Food Service Director/CDM and facility team regarding resident nutrition needs and concerns
  • Assist with regulatory compliance related to nutrition services and survey preparation
  • Maintain resident confidentiality and adhere to facility policies, procedures, and residents’ rights
  • Perform additional nutrition-related duties as assigned

Requirements

What you’ll need
  • Registered Dietitian credential through the Commission on Dietetic Registration (CDR)
  • State licensure/certification as required
  • Clinical nutrition experience
  • Skilled nursing or long-term care experience strongly preferred
  • Familiarity with nutrition assessment and care planning for geriatric and medically complex residents
  • Experience with electronic medical record/charting systems
  • Understanding of interdisciplinary care in a skilled nursing environment
  • Strong clinical judgment, organization, and time-management skills
  • Excellent written and verbal communication skills
  • Ability to work independently while collaborating effectively with the facility’s interdisciplinary team
  • At least 1 year of clinical experience preferred
