About the role
Key responsibilities & impact- Complete Comprehensive Assessment and Comprehensive Assessment Survey
- Complete GAIN-SS with all clients upon admission
- Complete Initial Services Plan with all clients
- Contact referents, when appropriate consent is in place, within the first three days of admission and weekly thereafter to provide progress updates and consultation regarding continuing care planning
- Assess clients for potential vulnerabilities using criteria in the Program Abuse Prevention Plan
- Develop, review and update Individual Abuse Prevention Plans as needed
- Develop comprehensive individualized treatment plans with specific and measurable goals, objectives and interventions within five sessions
- Update and revise treatment plans with clients according to the timeline outlined in 245G
- Document client participation and progress in client records according to Center policies and procedures
- Communicate regularly with the Outpatient Clinic Director and Mental Health Professional about client treatment and progress
- Facilitate group chemical dependency counseling and education sessions
- Develop and implement individualized continuing care plans, including coordination with significant others and referrals for medical, substance use, mental health and other services
- Support concerned persons, including family members and friends, with resources for sober support during and after treatment
- Complete professional development in Substance Use Disorders and Co-Occurring Disorders
- Contribute to the broader Center team, including general housekeeping, kitchen duties and light maintenance
- Perform other duties as assigned
Requirements
What you’ll need- Full or Temp licensure by the State of Minnesota for Alcohol and Drug Counselor in good standing
- Know and understand the implications of chapter 245G, chapter 245A, and sections 626.556, 626.557 and 626.5572
- Constantly operate a computer and other office machinery, such as a calculator, copy machine, printer, etc.
- Frequently communicate with employees, managers, patients and other individuals with key relationships
- Ability to receive and provide accurate information in these situations
- Frequently move around the office to access file cabinets, office machinery and supplies, etc.
- Must reside in Minnesota
- Full-time availability, 40 hours/week
- Group hours Monday–Thursday, 5p–8p
