About the role
Key responsibilities & impact- Connect with the person served utilizing telehealth equipment
- Provide support services including initial screening for crisis intervention, case management, assistance with treatment planning, client advocacy, prevention, and education
- Participate in virtual treatment team meetings and provide information pertinent to consumer treatment
- Educate and support consumers’ families
- Assist each consumer in obtaining Primary Care services
- Complete documentation in a timely manner
- Conduct ongoing consumer assessments and make appropriate changes in treatment plans
- Provide crisis assistance and coordinate with consumers, staff psychiatrists, family members, and hospitals/emergency units
- Provide proper paperwork for hospitalization
- Provide on-call services
- Maintain knowledge of core competencies for co-occurring disorders through continuing education and application in treatment
- Maintain confidentiality, inform consumers of their rights, and obtain authorizations for information release
- Provide clinical services teaching behavioral symptom-management techniques
- Perform other duties as assigned
Requirements
What you’ll need- 60 College credit hours in any field and 12 months of Behavioral Health experience, or High School Diploma or equivalent and 36 months of Behavioral Health experience
- Bilingual preferred
- Basic computer skills
- Current certification as a Behavioral Health Case Manager preferred
- Ability to stand or sit for long periods of time
- Ability to lift, push, or pull a minimum of 25 pounds
- Ability to travel locally approximately 10% of the time
- Ability to use telephone, PC, fax machine, copy machine, and printer
