About the role
Key responsibilities & impact- Assess physical, psychosocial and spiritual needs of the patient and family (RN only)
- Provide intermittent skilled nursing services under the patient's physician's direction
- Monitor and evaluate pain and symptoms
- Provide information and assistance to patients and families to manage pain and symptoms
- Serve as liaison to patients' physicians and provide relevant patient information
- Provide emotional support to patients and families
- Refer patients and/or family members to other services when appropriate
- Provide health care teaching to patients and families in their residence
- Visit the patient's residence at the time of death
- Maintain, promptly complete, and return required documentation
- Communicate and act as liaison with patients, families, and health professionals while respecting confidentiality and dignity
- Relay pertinent information to appropriate team members
- Participate in meetings initiated by the team leader
- Participate in mandatory or elective RC University events, trainings, or in-services
- RN's participate in triage coverage as needed
- Perform other duties as assigned
- Provide on-call nursing care after hours, typically during urgent situations, in patients' homes
Requirements
What you’ll need- High School Diploma or equivalent required
- Graduate of an accredited school of nursing
- Bachelor's degree preferred
- Licensed RN in state of Ohio
- Experience preferred in pain and symptom management, oncology, or mental health
- Ability to communicate effectively, orally and in writing
- Ability to work independently and collaboratively with a team
- Must have demonstrated knowledge and skills to provide appropriate care to all age groups
- Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere
- Ability to access patients and families in any residential and/or public setting in any weather condition
- Proficiency in Microsoft Office suite, particularly Word and Excel, shared team files, donor database, grant portals and virtual meeting technology
- Must have and maintain a valid driver's license, good driving record and auto accident insurance at an acceptable level
- Ability to stand and walk for extensive periods of time
- Manual and finger dexterity and eye-hand coordination to provide nursing care and operate equipment
- Ability to exert up to 75 lbs. of force occasionally and 50 lbs. frequently, using safe transfer techniques and body mechanics
- Ability to stoop, squat, or bend frequently
- Corrected vision and hearing within normal range
- Compliance with Annual Flu Shot Policy or ability to provide exemption documentation
- Active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage
- Active driver's license
- Proof of eligibility to work in the United States
- High level of integrity, ethics and professionalism
