About the role
Key responsibilities & impact- Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team
- Perform patient assessments, medication management, and care planning
- Educate and empower patients and families on disease management and treatment plans
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
- Work independently while collaborating with the support team
- Travel to patient homes within the assigned territory
Requirements
What you’ll need- Active Registered Nurse, RN license in the state you work
- One year of RN experience and the clinical expertise to deliver quality patient care
- Valid driver’s license
- Ability to spend approximately 20% of your workday driving to and from patient locations
- Successful completion of a drug screen prior to employment
- Legally authorized to work in the United States
