About the role

Key responsibilities & impact
  • Complete initial, comprehensive, and ongoing patient and family assessments to determine hospice needs
  • Obtain accurate demographic information and medical history, including diagnosis and primary care physician information
  • Introduce the agency to patients and caregivers and explain hospice and skilled services
  • Coordinate start-of-care visits
  • Order DME, supplies, and medications related to hospice care
  • Deliver community education to assisted living facilities, skilled nursing facilities, and hospitals
  • Provide bedside education about hospice, disease progression, and end-of-life care
  • Attend discharge planning meetings and support safe discharge home
  • Determine hospice eligibility and appropriate level of care
  • Communicate with the clinical team regarding complex, chronically ill patients to help prevent re-hospitalizations
  • Partner with field staff and leaders to support patients

Requirements

What you’ll need
  • Registered Nurse License is REQUIRED for this position.
  • Associate degree from an accredited School of Nursing
  • Minimum of two (2) years of experience working as a Registered Nurse, including acute care or oncology experience
  • Hospice experience preferred
  • Experience with Microsoft Office Suite, EPIC, and HCHB preferred
  • Able to transfer patients as needed according to proper techniques of body mechanics
  • Ability to accept different lifestyles, cultures, beliefs, and values
  • Proficiency with computers
  • Valid driver’s license required
  • Ability to spend approximately 20% of workday driving to and from patient locations
  • Successful completion of a drug screen prior to employment
  • Legally authorized to work in the United States
