About the role
Key responsibilities & impact- Provide direct support and mentorship to youth in the LTFC program
- Conduct routine calls to foster parents to coordinate community extracurricular activities, academics, and independent living skills
- Conduct home visits with youth and foster families as needed
- Co-facilitate Family Team Conferences with the clinician and attend Treatment Team Meetings
- Assist with community-based programming applications and referrals
- Accompany or provide telephonic support to foster parents and youth for school enrollment, appointments, and community resources
- Complete a Life Skills Assessment for each youth within 30 days of admission
- Facilitate individual and group Life Skills training
- Create and update the Independent Living/Life Skills curriculum
- Identify and utilize community resources, partnerships, and local out-of-network placements
- Conduct outreach with local community agencies to ensure culturally and linguistically appropriate resources
- Document all services within 72 hours
- Oversee minors' progress in care and share updates and recommendations with the treatment team
- Assist program staff with stabilization, teach social skills, and address behavioral issues with the clinician
- Perform other related duties as assigned
Requirements
What you’ll need- Bachelor's degree in education, psychology, sociology, or other relevant behavioral science preferred
- Two years' experience in a related program for the adolescent population
- Bilingual Spanish required
- Knowledge of child welfare and immigration-related legislation
- Excellent verbal, written communication, and organizational skills
- Proficient in Microsoft Office suite
- Knowledge of Connections
- Must complete Therapeutic Crisis Intervention (TCI) training
- Capability to establish and maintain working relationships with staff, community groups, and community-based organizations
- Understanding of practice models such as FTC, PBIS, Trauma-Informed Care, and evidence-based models a plus
- Valid driver's license from the tri-state area (NYS preferred) with a clean driving record
- Authorized to work in the U.S.
- Ability to work in-person in New York City (NYC)
- Ability to travel to other Rising Ground Sites if required
- Ability to work independently and as part of a team in a fast-paced, dynamic, and challenging environment
- Ability and willingness to work overtime and on weekends as required
- Must be able to travel extensively throughout the Tri-State area
