About the role

Key responsibilities & impact
  • Conduct intakes, mental health assessments, screenings, Individual Service Plans (ISP), case management, and therapeutic services for program participants
  • Oversee and implement care and resources addressing residents’ physical, emotional, educational, and social needs
  • Provide counseling, case management, and support to active participants
  • Provide weekly documented case notes for sessions
  • Review treatment and Individual Service Plans to ensure they meet youth needs
  • Complete intakes, case notes, Bio Psycho Socials/assessments, and referral materials
  • Provide clinical counseling for youth requiring intensive counseling or with possible mental health needs
  • Complete housing applications as participants transition from RHY programs
  • Participate in weekly Huddles and Personal Development meetings with program managers and participants
  • Facilitate or co-facilitate weekly workshops and groups
  • Enter data accurately and timely in PTS and other designated systems
  • Liaise with service providers supporting youth
  • Track services and referrals for monthly and quarterly reports
  • Conduct outreach and presentations to recruit youth to Drop-In Centers or TIL programs
  • Perform other relevant duties as delegated

Requirements

What you’ll need
  • MSW degree required
  • LMSW or LCSW licensing preferred
  • Authorized to work in the U.S.
  • Ability to work in-person in New York City (NYC)
  • Ability to travel to other Rising Ground Sites (if required)
  • Evening and weekend hours may be required
  • This position is in a 24/7 program
