About the role
Key responsibilities & impact- Identify potential admissions and process and place new resident/client requests
- Track admission status
- Complete authorization activities and determine accepting facilities’/agency’s capabilities to provide the appropriate level of care
- Support facilities/agencies by completing resident/client admission-related tasks
- Collaborate with colleagues and referral sources to plan, implement, and evaluate the admissions process
- Coordinate resident/client move-in processes, including admission paperwork, insurance verifications, prior authorizations, and transportation when applicable
- Ensure smooth transitions from referring facilities to locations
- Perform clinical evaluations of resident/client charts to determine appropriate medical services
- Determine appropriate actions for referring cases that do not meet facility/agency standards
- Visit with discharge planners, family members, and interdisciplinary team members to promote locations and services
- Comply with applicable laws, regulations, and policies/procedures
- Educate healthcare team members about trends, external regulations, and internal policies affecting resource utilization and prior authorization
- Enter data accurately into Customer Referral Management (CRM) and electronic medical record (EMR), depending on location
- Serve as a resource and point of contact for guidance, training, and questions
Requirements
What you’ll need- Appropriate education level required in accordance with state licensure
- Bachelor's degree from an accredited school and/or program is preferred, but not required
- Prior work experience in long term care facility preferred
- Currently holds an unencumbered North Dakota license: Licensed Baccalaureate Social Worker (LBSW), Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW)
- Ability to deliver high quality customer service in a professional manner
- Strong interpersonal and listening skills
- Ability to handle confidential and sensitive information
- Ability to coordinate several different responsibilities at once
