About the role
Key responsibilities & impact- Serve as a bridge between underserved communities and health and social service systems
- Work within the patient-centered medical home model and support chronic disease self-management
- Provide information and assistance to help patients navigate health and human services systems
- Advocate for patients and community groups and raise awareness of community health needs
- Build capacity for access to services and community engagement
- Engage patients in their own care using culturally sensitive, non-judgmental communication
- Collaborate on goal setting and care planning within an inter-professional team
- Give presentations to community groups
- Coordinate appointment attendance and individual plans of care
- Serve as a point person for issues and related solutions while maintaining appropriate boundaries
- Document assigned activities and support of community members
- Provide services under supervision as directed by state or regulatory requirements
- Adhere to patient privacy laws and the scope of duties; do not provide medical advice
Requirements
What you’ll need- High school diploma or equivalent required
- College courses strongly preferred but not required
- Preferred: minimum of three years of ambulatory care experience
- Experience in community outreach and/or working knowledge of various integrated systems of care in the community preferred
- Personal experience living with a chronic health condition preferred
- Motivational Interviewing skills required, with training to be completed within six months of hire
- Proof of certificate from a Community Health Worker program required upon hire or within six months
- Valid driver’s license required
- Strong communication, active listening, organization, coordination, public speaking, and presentation skills
- Knowledge and experience of ethnic and cultural populations, government partners, community stakeholders, and other organizations
- Thorough understanding of self-efficacy theory and its application when working with people with chronic disease
- Ability to adhere to patient privacy laws and scope-of-duty requirements
- Ability to work under supervision and avoid providing medical advice
