About the role
Key responsibilities & impact- Serve patients as an integral member of a Patient-Centered Medical Home or Primary Care Clinic
- Provide behavioral and mental health support, chemical health services, psychosocial care, and lifestyle management
- Conduct diagnostic assessments, onsite crisis assessments and interventions
- Determine need for ongoing care based on clinical ability and medical and psychosocial complexity
- Provide counseling using multiple modalities, from very brief to long-term
- Make appropriate referrals and provide educational services across the continuum of care
- Follow up with specialty-care referrals and ensure appropriate behavioral health management
- Serve as the primary team consultant on health and psychological/behavioral factors
- Lead universal screening for behavioral and chemical health conditions
- Collaborate with team members to deliver quality care
- Provide staff education and training on behavioral emergencies and behavioral health information
- Deliver empirically validated short-term behavioral health services, including assessment, consultation, diagnosis, and treatment
- Provide face-to-face and telehealth care to ambulatory primary care patients
- Address psychosocial, cultural, developmental, economic, familial, and religious factors affecting health and wellness
- Assist with population health management, cost and quality tracking, and integrated care model implementation
- Evaluate behavioral health care quality and effectiveness and participate in quality improvement activities
- Document care responsibly, accountably, and ethically
- Leverage multiple resources to support patients with medical and co-morbid behavioral conditions
Requirements
What you’ll need- Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences required
- Graduation from an accredited school preferred
- License must be in good standing in state(s) of practice
- In Minnesota, must have one of the following: Licensed Professional Clinical Counselor (LPCC) or Licensed Independent Clinical Social Worker (LICSW)
- Credentialing through the Allied Health Staff of the organization where you will be working will be required
- Knowledge of human behavior, legal and ethical issues, insurance regulations, behavioral health referrals and programs
- Knowledge of medical care, emergency services, and ambulatory care center operations
- Ability to provide coverage across all potential age populations and patients with co-morbid medical conditions
- Skills in communication, flexibility, team-based care, and decision making
