About the role
Key responsibilities & impact- Provide care to assigned hospice patients in a caring, safe, and efficient manner
- Evaluate outcomes using clinical, financial, and utilization data
- Define the unit of care as the family and patient
- Demonstrate knowledge of hospice palliative care, hospice philosophy, and scope of services
- Recognize cultural attitudes and values toward death and dying
- Identify the five stages of the grieving process
- Explain advance directives, clinical requirements, and available interdisciplinary services
- Use independent nursing judgment when integrating health care
- Participate in creating plans of care and ensure implementation by the interdisciplinary group
- Interview potential residents and assess residents for hospice admission and continued stay
- Establish and continuously evaluate service plans
- Provide teaching adapted to patient age groups
- Facilitate groups and implement projects when necessary
- Lead staff and collaborate with patients, families, and the health care team
- Document care, solve problems, and manage priorities according to location standards and individualized care plans
Requirements
What you’ll need- Bachelor's degree in Nursing strongly preferred
- Graduate from an accredited nursing program, including programs accredited by CCNE, ACEN, or NLN CNEA
- Minimum of two years’ clinical experience preferred
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in an NLC state
- Obtains and subsequently maintains required department specific competencies and certifications
- Must possess a valid driver’s license
- Must work primarily day hours, scheduled weekends, and occasional evenings
- Ability to communicate with patients/residents, family members and others on the health care team
- Ability to document clearly, solve problems, establish priorities, and work with little direction
- Knowledge of hospice palliative care, hospice philosophy, scope of services, reimbursement for services, and age-specific care methodologies
