About the role
Key responsibilities & impact- Coordinate an effective effort between the transplant center and major patient referral sources, including dialysis units and nephrology clinics
- Serve as case manager for potential transplant recipients and donors
- Coordinate timely transplant education, workup, updates, and activation for transplantation
- Manage a designated caseload during pre/post-recipient and pre/post-donor transplant processes
- Coordinate referrals and respond to roadblocks in the transplant workup process
- Provide educational programming for patients, families, and staff
- Ensure policies, procedures, and regulatory requirements are met for transplant program compliance
- Provide outreach services to regional dialysis and nephrology programs as needed
Requirements
What you’ll need- Graduate from an accredited nursing program required
- Bachelor's degree in nursing required at time of hire or within 5 years, or in the process of acquiring with an estimated completion date within five years
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact state
- Minnesota RN license required within six months of date into position
- Obtains and maintains required department-specific competencies and certifications
- Available for clinic hours Monday through Friday, 8am–5pm, with on-call expectations covering days, nights, weekends, and holidays
